If you are needing multiple reports for multiple contractors - you will need to fill out this form (STEP 2) for each contractor. However, you do not need to register (STEP 1) more than once. Ask about our multiple-reports discount!

This Contractor Authentication Report is only as good as the information we are given. The more information we have from you and/or the Contractor, the more accurate and complete our report. It IS of utmost importance that our report IS correct, not only for your sake, but in fairness to the contractor as well. We encourage you to share our report with your contractor. If either of you feels that corrections are needed, we can change our report for accuracy with the appropriate documentation.
